The footring is the whole point

Anyone sitting at a high bench on an ordinary stool ends up with their feet hanging, which cuts circulation at the back of the thigh and drives them to hook their heels on the base. A draughtsman stool puts a large footring at the right height for a raised seat, so the feet are properly supported and the weight is shared between seat and floor.

Contoured backrest

The backrest is shaped rather than flat, so it meets the lumbar curve instead of pressing on the spine. It adjusts for height, and because the person is seated upright at a bench rather than reclining, that contact point does most of the work of keeping the back neutral.
